FORM ONE REGISTRATION GUIDELINES – JULY 2024

REGISTRATION TAKES PLACE IN TWO (2) PARTS:

A. PHYSICAL REGISTRATION - to submit copies of all required documents as listed below

B. COMPLETION OF THE ONLINE REGISTRATION FORM 

PERSONS UNABLE TO COMPLETE THE ONLINE REGISTRATION AT HOME CAN DO SO AT THE SCHOOL.

YOU MUST COMPLETE BOTH A & B TO BE FULLY REGISTERED.

STEPS TO COMPLETING YOUR REGISTRATION:

1. Physical registration takes place on Thursday 4th JULY 2024 from 8:30 a.m. UNTIL 12:00 noon. 

All parents are required to attend this PHYSICAL REGISTRATION on the school’s compound:

TRANQUILLITY SECONDARY SCHOOL

5-7 Victoria Avenue 

Port-of-Spain

2. On Registration Day, you are required to walk with the ORIGINAL and COPY of EACH of the following documents:

1. Student’s Birth Certificate

2. SEA Placement Slip

3. Student’s Immunization Card

4. Student’s Performance Report

5. 2 Passport Size Photos of the Student. 

6. Parents (both if applicable)/ Guardian’s National ID or DP or Passport. 

DEADLINE FOR SUBMISSION OF MISSING DOCUMENTS TO THE SCHOOL’S OFFICE: FRIDAY 5th JULY 2024. 

The REGISTRATION FORM is available to complete ONLINE (see form below) if you have your personal device or when you come to the School on THURSDAY 4th July, 2024.

Deadline to complete the ONLINE REGISTRATION FORM: 

THURSDAY 4TH JULY 2024 AT 1:00 p.m. 

When entering the information, use the SEA Student Number as it appears on the SEA Placement slip. 

Note: 

• There will be computers available for use at the school on registration day to complete this form; however, we do encourage persons to complete this beforehand where possible.

3. Please register for the 2024 Vacation Revision Programme HERE. (Please note: Registration is not mandatory but can help with your child's transition from primary to secondary school)
